O'Connell Care at Home is looking for a **per-diem **Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our team at our Greenfield location serving the greater Franklin County area.

**Duties and Responsibilities (including but not limited to)**:

- Completes documentation timely, accurately, and at the point of care, according to industry standards, conventions and guidelines.
- Complete new client assessments, care plans, and updates as necessary.
- Completes in home demonstrations of personal care services for clients with community based caregivers
- Medication management and occasional skilled work with clients in the place that they call home
- Follows company policies and practices
- Performs additional related duties as assigned.

**Minimum Requirements**
- Possess a current and unencumbered Massachusetts license as a Registered Nurse (RN) or Licensed Practical Nurse (LPN)
- Graduate of an accredited LPN or RN program
- Minimum of two years' experience in the health care field; homecare preferred
- Possess a positive attitude, a willingness to learn, with a desire to be an effective member of the team
- Excellent communication skills and the ability to build meaningful relationships with internal and external stakeholders
- Comfortability using various forms of technology and software (Wellsky)
- Cognitive skills as related to the position.
- Thorough understanding of Private Case vs ASAP Cases and their requirements
- Due to the nature of our business or a myriad of other factors, employees may need to work from home. For that reason, access to reliable secure internet service at home is required.
- Reliable transportation, current driver’s license, and ability to drive to our various locations as well as the locations of our clients and stakeholders on a regular basis.
