If you are a compassionate Hospice RN Case Manager, we welcome you to join our cohesive and caring team of clinicians, part of a fast growing Home Health and Hospice company, and work in a territory close to your home.
**Your perks as one of our new Hospice RN Case Managers**:
- You will be supported by an experienced leadership team providing guidance and mentoring, and field support, as needed.
- You will work with a skilled clinical team, achieve reasonable leadership expectations while practicing best-practice standards and procedures.
- Your annualized FTE salary range is to $163K, based on your overall experience, plus a $9K relocation/sign-on bonus.
- As one of our new Hospice RN Case Managers, you will receive annual performance raises.

**Your responsibilities as one of our Hospice RN Case Managers**:
- A daily caseload of only 3-5 patients or an average of 15-20 per week
- Quite possibly, you will experience shorter commuting distances with us.
**You will bring the following skills as one of our new Hospice RN Case Managers**:
- Make a difference with quality patient care while honing your skills.
- At least 1+ years of current experience as a field Hospice RN Case Manager.
- Your stable work history, passion for working in a patient-centered environment, prior post-acute care experience, any related healthcare experience are all definite pluses to joining our cohesive clinical team as one of our new Hospice RN Case Managers.
