**General Summary**

**UMIT Home Health** is a Tribal owned home health agency offering both skilled nursing/therapy services and personal care aide services. We provide high quality, convenient, reliable in-home healthcare to patients. We service the following areas: Caroline Co., Charles City Co., Chesterfield Co., Colonial Heights City, Dinwiddie Co., Essex Co., Gloucester Co., Hanover Co., Henrico Co., Hopewell City, James City Co., King and Queen Co., King William Co., Middlesex Co., New Kent Co., Petersburg City, Prince George Co., Richmond Co., & Richmond City.

**Position**

Responsible for gathering patient information, verifying agency has a complete referral, registering patients in EMR, and sending complete referrals to scheduling. Works in person Monday - Friday and rotates weekend coverage.

**Primary Duties and Responsibilities**:

- Answers all incoming calls
- Review & coordinate incoming referrals
- Obtain & verify medical insurance coverage for patients
- Contact hospitals daily to confirm status of patients hospitalized
- Communicate effectively to all - patients, staff, and referral sources
- Report census data timely using Excel
- Perform clerical work, such as filing, copying, and faxing
- Rotates weekend intake coordinator coverage
- Receive, open, and put away deliveries

**Knowledge, Skills and Ability Qualifications**:

- High school diploma or GED
- Minimum two years’ experience in patient intake or medical office administration
- Home Health intake experience preferred
- Existing, strong home wifi required for weekend rotations
- Excellent organizational and time-management skills
- Superb customer service, interpersonal, & communication skills
- Functions effectively in open cubicle environment
- Optimistic, team-oriented attitude required
