**POSITION TITLE: LVN /RN (2P-10P SHIFT)**

***: The purpose of this position is to provide Nursing direction and Leadership to the Nursing staff for the 2-10 Shift. This position will start off as a "Charge/Floor Nurse" and will move into a "Nurse Manager" position. This position relies on the Director of Nursing (DON) for guidance.

**RESPONSIBILITIES**:

- Supervises Nurse Aides and Medication Aides; Coaches and disciplines as needed.
- Receives report on patient/resident status and evaluates and ensures resources are in place to meet needs.
- Makes physical rounds in assigned area of oversight assessing quality of care, intervening with provision of care and interacting with care givers and patients/residents.
- Provides direct care in line with the responsibilities of a nurse, assessing conditions, communicating with attending physicians/practitioners and carrying out nursing functions according to the needs of the patients.
- Accurately documents patient/resident condition including assessments, communication and care provided.
- Ensures safe work practices are followed.
- Provides empathetic communication to families, patient/residents regarding change of condition in response to care concerns, ensures resident rights are respected.
- Assists with the Continuous Quality Improvement initiatives.

**QUALFICATIONS**:

- Must be a licensed nurse LVN with active, applicable state licensure.
- Prior experience in healthcare/LTC preferred.
- Nursing experience in a long-term care facility is preferred.
- Supervisory experience is desirable.
- Effective communication and interpersonal skills are necessary.
- Ability to speak, read, write and understand English.
- A valid TB test and BLS/CPR card.
- Ability to lift at least 51 pounds on a regular basis and physically able to help residents who require physical assistance.
- Ability to pass a criminal background check and drug screen.

**OTHER QUALIFICATIONS**:

- Must possess a general appreciation of seniors and an understanding of the aging process, as well as be aware of its challenges.
