Assesses, diagnoses, plans and implements the medical care of patients in collaboration with a physician. Functions within the full scope of licensure; accountable for the assessment, coordination delivery and evaluation of nursing care, including direct patient care, patient/family education and transitions of care. Delivers holistic and individualized care to all patients in assigned area. Develops, implements, manages an optimal interdisciplinary plan of care that incorporates psychological, sociocultural, spiritual, economic, and life-style factors. **EDUCATION**
Graduate of an accredited school of nursing

Master's: Nursing

**CERTIFICATION & LICENSURE**
RN - Registered Nurse State Licensure

NP-Nurse Practitioner (National certification)

AANP or ANCC or PNCB National Certification
BLS-Basic Life Support Healthcare Provider

NPF-Nurse Practitioner Furnishing License
CSRC-Controlled Substance Registered Certification (DEA)

**SKILLS AND KNOWLEDGE**
Knowledge of community agencies and resources.
Knowledge of mandated reporting requirements
Verbal and written communication skills for frequent and challenging contact with physicians, employees, patients and patient families; requires patience and compassion.
Psychiatric assessment skills that can be applied to a wide range of DSM diagnoses.
Ability to provide brief psychotherapies as appropriate.
Ability to manage diverse caseload in a manner that meets patients’ needs while also providing the capacity for new patients entering treatment.
Interpersonal skills, and the ability to support change in a fast-paced and team-oriented environment.
Ability to provide care that is consistent with professional standards, clinic policy, and managed care guidelines.
