This position manages clients' cases from intake to discharge, follows clients' progress, and keeps Clinical Director informed on any issues/problems with any clients. The Case Manager is charged with coordinating on-going and ancillary care for the client that may include but is not limited to legal appointments, vocational issues, insurance coordination, external referrals for ancillary care. The Case Manager will document all of the client activities that s/he is responsible for in the client chart.

**Duties and Responsibilities**:
1. The Case Manager will maintain a caseload and be involved in client treatment processes:
a. Maintains contact with client's family on a weekly basis discussing progress/problems depending on client's release of information.

b. Assures proper client consents and that other treatment documents are completed.

c. Maintains contact with client on a daily basis, provides one-on-one sessions with clients one to two times a week if necessary to coordinate services.

d. Corresponds with client's probation officer and the courts if applicable.

e. Maintains contact with Clinical Director and Program Director regarding caseloads.

f. Completes individual case management assessment and treatment plan.

g. Assists with facilitating groups appropriate for level of certification/licensure as needed.

h. Responsible for attending and participating in mandatory weekly staff

meetings to discuss client care and all aspects of administration.

i. Knowledge of policy and procedure manual relating to program protocol.

j. Other duties as assigned.

2. Case Manager performs and maintains professional abilities in the following areas:
a. Maintain and adhere to all regulations regarding County regulations regarding client confidentiality and staff code of ethics.

b. Perform tasks the supervisor assigns including, but not limited to, schedule, case load, and deadlines.

c. Continuous training in areas where needed, attendance at seminars, continue earning CEUs.

d. Treatment planning is kept current and fits with the needs of the client.

e. Crisis intervention—recognizing emotional stress and utilizing resources to resolve problems.

f. Client education—provide current alcohol/drug information and available resources to clients, groups, and families.

**Qualifications**:
1. Minimum of one (1) year previous experience in the addiction field.

2. Ability to prioritize and provide appropriate treatment protocols.

3. Maintains current certification for this position:
a. CPR

b. First Aid

c. HIPAA/Confidentiality Training

Pay: $40,000.00 - $50,000.00 per year
