Social Worker

Brand New Adult Day Health Care Center (ADHC) is seeking a Social Worker to join our dynamic team The Social Worker will assume responsibility for a caseload of approximately 40 adult participants, most of whom have one or more medical diagnoses. The Social Worker will provide a range of services including case management, community resource referrals, counseling to elderly and other physically/mentally impaired adults in a day program setting.
**Requirements**:
- Bachelor’s degree in Social Work from an accredited school with 2 years of experience providing social services in one or more of the fields of aging, health, or long-term care services OR
- Master’s degree in Social Work from an accredited school of social work OR
- Master’s degree in Psychology, Gerontology, or Counseling from an accredited school and 1 year of experience providing social services in one or more of the fields of aging, health, or long-term care services
- Valid Driver's License and proof of insurance
**Responsibilities**:
- Conduct biopsychosocial assessments (includes home assessment)
- Case management (schedule appointments, coordinate transportation, assist with access to community resources, etc.)
- Develop and implement person-centered treatment plans, with the ability to determine appropriate milestones to gauge progress
- Provide individual and family counseling
- Attend multidisciplnary team meetings, communicate updates to relevant parties
- Make referrals to other healthcare providers as needed
- Complete all documentation and reports in a timely and accurate manner
- Community outings
- Group facilitation
- Marketing and outreach
- Intakes and discharges
Qualifications:
- Must be energetic, self-motivated, organized, and efficient with completing tasks and meeting deadlines
- Must be comfortable working in a culturally-diverse setting, and utilizing on-site translation services when necessary.
- Must have excellent oral and written communication skills, with basic proficiency in Microsoft Office (additional computer program training will be provided
- Must be willing to assume additional job duties as assigned; center staff operates as a team, and all staff fill in with additional tasks when necessary (assisting with activities, meal service, answering telephones, filing, etc)
- Knowledge of community resources in Modesto and surrounding areas a plus
- Bilingual preferred, but not required (Spanish, Assyrian)
