**Description**:
Join our dynamic community health team and play a crucial role in enhancing the well-being of our community. As a Community Health Worker, you will be instrumental in bridging the gap between underserved populations and healthcare services, focusing on health education, preventive care, and facilitating access to healthcare and social services.

**General Summary**:
This position promotes the improvement of the healthcare status in the community by facilitating health educational and wellness activities. Emphasis is on the importance of screening and early detection, and providing healthcare navigation with links to screening, treatment and primary care as needed.

This job takes into consideration actions needed to reduce the rate of death and disease due to ethnic healthcare disparities.

Cancer and other diseases relevant to improving health status will be targeted. All programs are conducted in a culturally and linguistically competent and sensitive manner. Programs are designed to meet the needs of the vulnerable and medically underserved as well as the broader population. Stewarding of resources is essential to the sustainability of this program.

Supports the Mission of Trinity Health and Holy Cross Hospital.

**Minimum Licensure/Certification Required (if applicable)**:
CPR if applicable

First Aid preferred

RN if applicable

**Minimum Knowledge, Skills, and Abilities Required**:

- 1-3 years of clinical or allied health experience in a medical environment preferred
- Bachelor’s degree with a concentration on community/public health or health education preferred **OR** Professional allied medical education preferred (i.e., U.S. allied healthcare degree, foreign schooling degree [minimum two years] or foreign licensure) preferred
- Able to provide health education, facilitate screening for early detection of disease, and link to treatment and care
- Excellent interpersonal skills that will facilitate the education and successful recruitment for early detection and screening
- Capable of instructing in their native language as applicable
- Able to evaluate teaching when appropriate (i.e., pre
- and post-test, repeat demonstration)
- Able to provide medical interpretation and translation as needed
- Able to recognize level and enhance medical literacy
- Able to obtain and responsibly collect relevant data to measure outcomes
- Computer literate with proficiency in word processing, spreadsheet creation and maintenance (Excel), electronic presentations (Power Point), and Internet software.
