**MUST HAVE INTAKE COORDINATOR EXPERIENCE**
**The Responsibilities of the Intake Coordination**:
- Proficient in Axxess /Kinser well sky EMR software.
- Schedule and staff all patient visits within prescribed discipline and visit frequency requirements.
- Verify the primary care physician who will assume responsibility for home health oversight.
- Ensures authorization for service is confirmed for all admissions, as required by payers.
- Ensures appropriate physician orders for referrals are obtained, as indicated.
- Work with community referral sources, families, and Agency staffs to problem solve and identity services available to the client either through Agency services or the community.
- Assist with reviewing of patient medical records, visit notes, physician orders for compliance with federal, state and local laws, Accrediting Organization standards and Agency policies and guidelines.
**The Qualifications**:
- Excellent customer service skills
- 0 -2 years of home health care experience required
- Must have a vehicle, driver’s license, and appropriate automobile insurance coverage.
- Communication, interpersonal, and organizational skills necessary to complete job Responsibilities
- Experienced with Axxess AgencyCore (EMR Software) Experience
- Home Healthcare: 2 or more years (preferred)
This position offers an excellent benefits package and a salary commensurate with

This is a full time position that includes a work schedule Monday

Pay: $19.00 - $30.00 per hour
