DESCRIPTION
Sun River Health, a Federally Qualified, Non-For-Profit Health Center serving communities in Suffolk County, provides the highest quality comprehensive primary, preventive, and behavioral health services to all who seek it, regardless of insurance status and ability to pay, especially for the underserved and vulnerable.
The Linkage to Care Specialist conducts targeted outreach, recruitment, linkage and care coordination to assist people in accessing screening and linkage to care for HIV/AIDS, Hep C, STIs, PrEP, mental health and/or substance use disorder. Promotes the program directly to the community, as well as other CBO's and medical providers. Additionally, can promote overdose prevention, including Narcan training and distribution, testing/screening for HIV, PrEP, STIs, Hep. C and submit monthly reporting.
Essential Duties, Responsibilities and Specialized Skills of a Linkage to Care Specialist I:
- Knowledge and experience working in the field of SUD, PrEP and/or HIV
- Conduct program intake including the social determinants of health, housing retention, risk assessment and service/action plans
- Provide health education, risk reduction and adherence education to patients
- Provide community testing and linkage to care
- Collaborate with the HIV, MAT, Medical and Behavioral Health Provider teams.
- Develop linkages and referral networks in the community for the purpose of referring patients for additional services and support
- Possess excellent communication skills in both verbal and written form
- Possess the ability to demonstrate cultural sensitivity in working with diverse populations
Minimum Education Requirement:
Bachelor’s degree
Minimum Specialized Skills & Knowledge Requirement:
Bilingual English/ Spanish speaking strongly preferred
**Salary**: $22-27/hour
