We are currently looking for a motivated, passionate RN to have a Full-Time RN Caseload, managing the medical needs of terminal patients and provide quality care. This case manager would operate Metropolitan Phoenix Area and within facilities in the West Valley and Scottsdale (clumped visits in one place within phenomenal facility partners).

This RN will start out with a moderate caseload of about ten patients which will grow as we grow but will not exceed 15 patients at a time. Utilizing their depth of experience and knowledge, the home health nurse is responsible for:

- Completing routine hospice comprehensive assessment of patient and family to determine home care needs
- Conducting in home visits as assigned, assessing the patient’s condition, progressing towards goals, providing patient care, teaching and other skilled interventions as ordered by a physician
- Evaluating patient care needs
- Administering medications and treatments as prescribed by the physician

**Requirements**:

- Valid AZ RN license (or nurse compact license) (_required_)
- Hospice experience of 1-3 years _(preferred)_
- Experience with Wellsky Consolo (_preferred_)
- Valid AZ Driver’s License and automobile insurance (_required_)
- Reliable transportation (_required_)
- Pass background check (_required_)

Pay: $80,000.00 - $90,000.00 per year

Schedule:

- 8 hour shift
- Monday to Friday

**Experience**:

- Hospice: 2 years (required)
- Nursing: 5 years (required)

License/Certification:

- RN (required)
- BLS Certification (required)

Ability to Relocate:

- Phoenix, AZ: Relocate before starting work (required)

Willingness to travel:

- 50% (preferred)

Work Location: On the road
