Memory Care Coordinator

The Memory Care Coordinator is responsible for developing, coordinating, and implementing programs and services for residents with cognitive impairment; to provide a therapeutic environment that maintains each resident's highest level of physical, social, and psychological well-being. The Memory Care Coordinator will work at least 30 hours per week.:
**You will**:
- Research create and execute the daily activities for residents with Dementia.
- Creates consistently high-level activity programs that reflect the interests and capabilities of the residents we serve.
- Assesses the interests and activity needs of our residents at the time of move-in and throughout their residency with us. Orient all residents to the activity program and how to access activities.
- Coordinates and assist with resident outings.
- Assists in planning and developing monthly activities calendar and newsletter.
- Observes problems, concerns, issues with employees and/or residents and communicates them appropriately to the Executive Director and Director of Nursing.
- Records resident information as required.
**You currently**:
- Associate or Bachelor degree combined with ongoing certifications and/or continuing education credits in geriatrics, Dementia, social service, leadership or related, strongly preferred.
- 2+ years of experience in a Dementia care setting preferred.
- 2+ years of experience planning, coordinating, and leading activities and events.
- Above-average computer skills.
