**JOB DESCRIPTION**

**CHAPLAIN**

I. QUALIFICATIONS:
A. High School graduate with two (2) years of college credit preferred.

B. One (1) year experience working with terminally ill patients preferred.

C. Training and experience in counseling, pastoral care services and program development.

II. JOB KNOWLEDGE:
A. Understand the Hospice concept and philosophy of care.

B. Ability to guide and assist patients and families as well as team members in spiritual and/or general counseling situations.

C. Ability to work in a non-denominational environment.

D. Ability to be compassionate and communicate effectively, utilizing appropriate listening skills.

E. Ability to facilitate individual and group counseling.

F. Ability to respond and function appropriately in crisis situations.

G. Ability to conduct educational seminars and training for civic, church, volunteer and professional groups.

H. Understand the complexities of theological and spiritual issues of hospice care, death, dying, and grief.

I. Ability to work consistently under pressure; to be objective; to keep feelings, prejudices, and personal problems from having a negative impact on work performance.

III. FUNCTIONS:
A. Visits patients/families, providing direct spiritual counsel in conjunction with the plan of care and according to the Chaplain Standard Operating Procedures. Conducts initial spiritual assessments, routine visits, post transfer visits, KPS visits, and PRN visits in the home, nursing facility, hospital, or inpatient setting.

B. Must follow all infection control and COVID-19 policies.

C. Coordinate with other chaplains to ensure that all patients receive a case-load assignment and are seen timely, and effectively maintain personal case-load to ensure visits are completed within Standard Operating Procedure timeframes.

C. Documents all visits and phone calls in the clinical record. All documentation must be incorporated into the medical record within 72 hours of visit.

D. Contacts patient's clergy of choice to collaborate and strengthen support as requested and authorized by the patient/family.

E. Serves as a member of the interdisciplinary team, contributes to the plan of care and collaborates with the other members of the team in order to obtain goals and objectives.

F. Serves as a spiritual and emotional resource to hospice staff and volunteers assisting them in grief, loss, and in coping with work related stress.

G. Conducts staff training programs and continuing educational programs as needed.

H. Complies with the quality assurance program.

I. Attends patient care conferences and other meetings as requested.

J. Available to assist with funerals and memorials

K. Provides support to family members in the period of bereavement in conjunction with other team members as requested.

L. Rotates on-call with other chaplains.

M. Completes daily activity sheets.

N. Maintain confidentiality and upheld privacy practices.

O. Fully complies with all Hospice in the Pines Policies and Standard Operating

Procedures.

P. Provides spiritual care (chaplain visits) to the palliative care program as requested

Q. All other duties as assigned.

IV. ENVIRONMENTAL AND PHYSICAL REQUIREMENTS FOR CLINICAL

POSITIONS (to perform essential functions):
A. Must be able to work in diverse settings, to include possible contact with blood borne pathogens, respiratory hazards, and electrical hazards.

B. Complies with COVID-19 infection control policies.

C. Must be able to work in potentially volatile situations, such as being verbally confronted by hostile individuals, patients, families, to include derogatory language.

D. Must be able to work alone doing repetitive tasks for extended periods of time to include standing.

E. Must be able to work in high traffic area without disruption.

F. Must be able to stand, walk, lift, pull, and push.

G. Must be able to speak English, have good near vision, hear and smell for sensory purpose.

H. Must be able to distinguish colors to verify proper medical condition for assessment purpose.

I. Must be able to climb stairs and ladders, for job requirements.

J. Must be able to perform repetitive motions, stooping, bending, grasping, reaching, kneeling, carrying, and typing/ word processing.
