Los Robles Healthcare is seeking a dedicated and compassionate LVN Clinical Liaison to join our dynamic team.

The LVN Clinical Liaison will play a crucial role in ensuring the quality of care provided to our home health patients by conducting quality assurance visits. The primary focus will be on assessing patient needs and ensuring that the appropriate service lines are tailored to meet those needs.

**Responsibilities**:

- Conduct quality assurance visits for home health patients to assess the quality of care being delivered.
- Evaluate patient needs and collaborate with the interdisciplinary team to ensure the proper service lines are fitted to meet those needs.
- Provide education and support to patients and their families regarding their care plans and available services.
- Communicate effectively with physicians, nurses, therapists, and other healthcare professionals to coordinate care plans.
- Document all interactions and findings accurately and in a timely manner.
- Participate in interdisciplinary team meetings to discuss patient progress and care plans. Identify areas for improvement and work collaboratively with the team to implement quality improvement initiatives.
- Serve as a liaison between patients, families, and Los Robles Healthcare to ensure continuity of care and patient satisfaction.
- Stay current on industry trends, regulations, and best practices related to home health care. Qualifications: Licensed Vocational Nurse (LVN) in the state of CA.

Pay: $28.90 - $34.99 per hour

Expected hours: 40 per week
