Responsibilities:_**
- Conduct a complete nutrition assessment and determine program eligibility.
- Provide and document participant-centered nutrition education (individual/family sessions and facilitated group discussions) responsive to the identified needs/interests of participants.
- Offer and document appropriate follow up referrals to other health and social services agencies.
- Implement care plans for participants at nutritional
- Guide CPA’s and nutrition staff in nutrition assessment and counseling.
- Track participants’ progress and document outcomes.

**_ Qualifications:_**
- Bachelor’s Degree in Nutrition.
- Bi-lingual Spanish/English.
- 1-3 years related experience.
- Proficient with Microsoft Office.
- Experience with data entry.
- Demonstrated excellent customer service skills.
- Clean, valid NYS Driver’s License.
