Job Title: LPN, Long Term Care (LTC)

Job Summary:
Our long term care facility is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team. The LPN will be responsible for providing quality care to residents in our LTC facility, collaborating with healthcare professionals, and ensuring the overall well-being of our residents.

Responsibilities and Duties:
1. Administer medications and treatments as prescribed by healthcare providers.

2. Monitor residents' vital signs and assess their health status.

3. Provide hands-on nursing care, including assisting with personal hygiene, mobility, and activities of daily living.

4. Communicate effectively with residents, their families, and healthcare team members.

5. Implement care plans and ensure residents receive appropriate care and support.

6. Document all nursing interventions and maintain accurate medical records.

7. Participate in care conferences and provide input on residents' care plans.

8. Adhere to state and federal regulations governing LTC facilities.

Qualifications and Skills:
1. Valid LPN license in the state of practice.

2. Minimum of 1 year of experience in long term care or a similar setting.

3. Strong clinical skills and knowledge of nursing best practices.

4. Excellent communication and interpersonal skills.

5. Ability to work collaboratively with a multidisciplinary team.

6. Compassionate and patient-centered approach to care.

7. Ability to prioritize tasks and work efficiently in a fast-paced environment.
