**PURPOSE OF POSITION**
The Peer Advocate is culturally aware and sensitive to the needs of the targeted population, possess excellent computer skills, be able to function as a part of a multidisciplinary team and understand therapy and group work rooted in evidenced based approaches for intervention and management strategies. Peer Advocate services are peer-delivered services with a rehabilitation and recovery focus. They are designed to promote skills for coping with and managing behavioral health symptoms while facilitating the utilization of natural resources and the enhancement of recovery-oriented principles (e.g., hope and self-efficacy, and community living skills). Peer Advocate support uses trauma-informed, non-clinical assistance to achieve long-term recovery from substance use and homelessness..

**RESPONSIBILITIES**
The major responsibility of this position includes, but are not limited to:

- Help promote skills for coping and managing behavioral health symptoms
- Educates clients about Medication Assisted Treatments (MAT), prevention, harm reduction, and Narcan.
- Work as a member of multidisciplinary treatment team that practices client centered and holistic care.
- Accompany clients to appointments and complete warm handoff referrals to SUD.
- Communicate effectively with clients and collateral contacts.
- Conduct outreach for care coordination.
- Assist in the selection and utilization of self-directed recovery tools such as relapse prevention planning.
- Connect individuals to self-help groups in the community.
- Provide recovery education and counseling to individuals.
- Provide preventative and crisis support services.
- Conduct support groups to strengthen social skills, decrease isolation, provide emotional support, and create opportunities for ongoing natural support.
- Participate in staff and organizational meetings as requested for care coordination.
- Maintain compliance with all organizational policies and procedures.
- Complete wellness and health promotions activities.
- Follow-up with the individual to confirm linkage to Care Coordination, outpatient treatment or other community services.
- Perform additional relevant duties as requested by supervisor/management.
- Conducts outreach at various locations throughout NYC.

**EDUCATION**
High School Diploma or GED, and one year of applicable experience and/or equivalent experience

**QUALIFICATIONS**
- Previous professional or personal experience in housing placement and assistance preferred
- Previous experience delivering evidence-based interventions such as, RESPECT & D-Up
- Previous work experience with high risk, Harm Reduction and HIV+ populations.
- Outstanding writing and communication skills; including multi-media outlets.
- Work experience with homeless individuals in a medical/health or social service settings
- Experience in community based medical/health or social service settings, social work, counseling, nursing, public health, or other health education.
- Knowledge of citywide resources and referral processes for homeless individuals
- Minimum five (5) years of similar work experience, facilitating groups and performing Outreach/community engagement activities.

**Compensation & Benefits**
- The base pay range for this position is $35,000 - $40,000 per year. *
