**HCBS Care Coordinator - Community and Home Based Services**

**JOB SUMMARY**:
Oversees the daily operating function of the organization operational management of our HCBS programs (Attendant Care, Habilitation, Homemaker, Respite and IDLA Services) by implementing the agency’s mission, vision, and values.

**Compensation**: $16.00 - $17.00/hr.

**Hours**: 8 am - 5 pm

**Status**: Fulltime: non-exempt

**Responsible for the Agency which includes**:

- Manage day-to-day operations of iCare Health Services HCBS program including organization and implementation of direct support professionals, person-centered planning for the members, and the implementation, retention, and maintenance of the members.
- Manage the implementation of program planning: monitor delivery of services and supports to ensure services are consistent, engaging, and relevant to the members served.
- Ensure all services quarterly reports are submitted to DDD on time - vTrack can help with this Responsible for the Employee which includes:

- Recruit, train, onboard, and manage new Direct Support Providers (DSP)
- Determine applicant qualifications by interviewing applicants; analyzing responses; verifying references; comparing qualifications to job requirements.
- Assist with identifying and matching direct support providers to members, and provide ongoing supervision and training for DSPs
- Manage HCBS program staff members including performance evaluations, scheduling, and orientation
- Maintain accurate records on providers’ status and overall new provider management
- Ensure background central registry check is submitted and filed for every DSPs
- Ensure that staff maintain compliance regarding:**Article 9Prevention and SupportFingerprint ClearanceCPR & 1st AidDCW (1 &2)**:

- Ensuring staff providers are trained on the EVV clock in and out requirements - all HCBS DSPs are to clock in and out.
- Ensure all DSP providers completes their daily service documentation accurately, submits their timesheet on-time with parent/guardian’s signature and within designated time frames.
- _**Occasionally the HCBS Coordinator may need to fill-in and provide care for members if a shift cannot be filled as originally scheduled**_.
- Responsible for the Members which includes**:Responsible for interacting with and coordinating care for the families we serve, completing periodic check-in home visits - Supervisory visit required by DDD for the 5days, 30days, 60days (if needed), and 90 days home visits.**:

- Responsible for attending member quarterly PCSP team meetings (in person and/or virtually) and conducting service reviews; etc.
- Communicate with parents and/or providers regularly to ensure service objectives are being met
- Provide telephone, virtual and in person support as needed for members, families, and providers
- Demonstrate conflict resolution by handling complaints and/or concerns from families or providers.
- Maintain accurate records on member’ status and overall member management Review and submit incident reports as needed
- Focus on members growth and potential by ensuring reliable services are utilized
- Performs other related duties as assigned
- You will not be able to have your set schedule to work with members due to you being on call for other providers.

**Skills required for the position**:

- Must have at least six (6) months experience in conducting group or individual activities related to specific developmental, habilitative, or recreational programs.
- High School Diploma
- Experience supporting and interacting with individuals with developmental disabilities.
- Knowledge of various computer systems, particularly Excel and Word.
- The ability to work in partnership with other professionals, including case managers, medical staff, etc.
- Excellent verbal and written communication skills.
- Excellent organization skills and the ability to multi-task.
- Excellent time management skills
- Must have the ability to work independently and be self-directed.
- Must maintain professionalism at all times while representing the company
