**Job Summary**
**Responsibilities**
- Coordinate with colleagues within the office and field to assess, plan, implement, or evaluate patient care plans.
- Ensure all orders are received and processed timely for patient delivery.
- Communicate effectively with hospice team regarding patient needs and facilitate multiple care aspects.
- Participate in hospice IDT meetings as part of an interdisciplinary team of Nurses, Physicians, Social Workers and Chaplain.
- Record information with accuracy and produce reports as needed.
- Provide compassionate and caring interaction with patients/families to ensure satisfaction.
- Promote high quality and cost-effective interventions and outcomes.
- Maintain accurate records of patient care and discuss observations with the supervisor.
- Adhere to professional standards as outlined by protocols, rules and regulations.
**Qualifications**
- Currently licensed as a Vocational Nurse in state of California
- Previous experience as an LVN case manager is preferred
- Technical skills needed for Microsoft, Excel, Google Docs and EMR system
- Excellent interpersonal and problem solving skills
- Strong organizational and time management skills
- Ability to multi-task and work as part of an interdisciplinary team
- Compassionate and able to discuss difficult topics related to end-of-life
- Determined and interested to learn and grow with the hospice
