**Job Qualifications**:
**Education**:Master’s Degree Nurse Practitioner

**Licensure**:State licensed as a Master’s Degree Nurse Practitioner; State Driver’s License

**Experience**:Previous experience in palliative care, home health, geriatrics or general medical surgical nursing.

**Skills**:Nursing skills as defined as generally accepted standards of practice. Strong communication and interpersonal skills; Sensitivity to issues of loss and grief; Ability to deal effectively with the demands of the job.

**Transportation**:Reliable transportation.

**Health**:CPR certified; TB health screen or TB test/CXR according to policy; Hepatitis B Vaccination Consent/Declination

**Environmental and Working Conditions**:
Works in patients home in various conditions; ability to work flexible schedule; ability to travel locally; some exposure to unpleasant weather; On call. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of the position without compromising the quality of work.

**Physical and Mental Effort**:
Prolonged sitting and walking required, with ability to lift up to 50 lbs. and move patients. Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition; meet patient/family individualized psycho social needs. Requires hand-eye coordination and manual dexterity.

**Essential Functions**:
Promote hospice mission, goals and philosophy.

Demonstrate commitment, professional growth and competency.

Demonstrate compliance with policies, procedures, regulations defining scope of practice.

Promote security and safety of Protected Health Information.

Participate in programs and on committees as requested.

Participate in ongoing education as required.

Perform face to face encounters and assessments upon request and in timely manner.

Educate patient/caregiver as indicated.

Communicate visit findings with Hospice Medical Director, as necessary.

Document services and submit timely.

Other duties as assigned.

Pay: From $115,000.00 per year
