Position: Visiting Nurse

Location: Peoria, Surprise, Glendale, Phoenix, Scottsdale, Mesa, Gilbert, Apache Junction, Maricopa County region

**Key Responsibilities**:

- Conduct comprehensive assessments of patients' conditions and needs.
- Develop and implement individualized care plans.
- Administer medications and treatments as prescribed by physicians.
- Provide emotional and psychological support to patients and their families.
- Collaborate with a multidisciplinary team, including physicians, social workers, and chaplains.
- Educate patients and families on the hospice care process and end-of-life issues.
- Maintain accurate and timely documentation of patient care.
- Participate in on-call and holiday rotation as needed.

**Qualifications**:

- Valid Registered Nurse (RN) license in Arizona.
- Minimum of 2 years of nursing experience, preferably in hospice or palliative care.
- Excellent clinical, organizational, and communication skills.
- Ability to work independently and as part of a team.
- Compassionate and empathetic demeanor.
- Strong critical thinking and problem-solving abilities.
- Current CPR certification.
- Valid driver's license and reliable transportation.

Pay: $65.00 - $75.00 per hour
