The Role

Ideal candidates are those who thrive working with a collaborative and compassionate community of caregivers. As a Nurse Practitioner at Greater Good Health, you'll take the lead in patient care at our primary care clinic serving the Greater Idaho Falls area. The role will be responsible for delivering an exceptional patient experience by working closely with the care team to provide holistic, high-touch, coordinated care for a panel of senior patients. The Nurse Practitioner will be responsible for managing a patient panel, supported by the Medical Assistants/LPN's, Front Office Coordinator, Clinic Manager, Medical Director, and additional teammates as we grow.

• Open to feedback from the clinical and leadership team to strive for constant improvement with coding and processes
• Comfort with technology systems, commitment to timely EMR documentation, and have a deep understanding of Medicare HCC coding and managed care
• Flexible with work schedule and work setting as needed
• Ability to travel within the 30 mile radius from Idaho Falls, ID 5 days a week to provide care to patients based on the needs of business/market

Experience and qualifications

• Active Idaho Nurse Practitioner license
• Practice in Primary Care and independent patient care 3-5+ years (Required)
• Acute care experience with high-risk patients is a plus
• Home health experience (NP or RN) is a plus
• Nurse Practitioner certification from either the ANCC / AANP
• Adult Gerontology specialty board certification (Preferred)
• Value-based panel management and care coordination experience (Preferred)
• New practice implementation and growth experience (Preferred)
• Prescriptive Authority by the ID state board of Nursing or Medical Examiners and DEA License (Required)
• Working knowledge of chronic care management and various medication options
• Current DEA licensure / Prescriptive Authority post-hire
• Active CPR/BLS Certification
• Current Idaho Driver's License
