Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to understand clients' mental health concerns, strengths, and resources, ensuring that each treatment plan is personalized and effective. \n
• Provide individualized, family, and group therapy using various evidence-based treatment modalities, including cognitive-behavioral therapy, solution-focused therapy, and trauma-informed care. \n
• Develop, implement, and periodically review individualized treatment plans, establishing achievable goals and interventions tailored to each client's unique situation. \n
• Collaborate with the treatment team to coordinate care, ensuring a comprehensive approach that addresses all aspects of a client's well-being. \n
• Facilitate dynamic group therapy sessions and psychoeducation classes, enhancing community support among clients. \n
• Conduct crisis interventions, risk assessments, and safety planning as necessary to address immediate needs and ensure client safety. \n
• Maintain accurate and timely documentation, aligned with ethical and legal standards, to support both clinical and administrative functions. \n

This role focuses on individuals who are experiencing a range of mental health challenges, particularly those related to hospice care. The clients you will work with are navigating significant emotional distress related to terminal illness, loss, and transition. Your clinical expertise will be vital in providing the necessary support, promoting healing, and facilitating coping strategies to improve their mental health outcomes.

Qualifications\n

• A Master's degree in Social Work from an accredited program. \n
• Active state licensure as a Licensed Clinical Social Worker (LCSW). \n
• A minimum of two years of experience in a mental health or outpatient setting, preferably with hospice or palliative care knowledge. \n
• Strong familiarity with evidence-based therapeutic modalities and assessment tools. \n
• Exceptional interpersonal and communication skills, with a talent for building rapport and supporting clients through difficult times. \n

Compensation and Benefits\n
