Position OverviewThrive Compassionate Counseling is seeking a committed and skilled Licensed Clinical Social Worker (LCSW) for a full-time position within our Detox Center located in Chicago, Illinois. In this role, you will play a vital part in supporting our patients as they navigate the complexities of their rehabilitation journey. Work EnvironmentThis position is fully remote, allowing you to deliver critical support from the comfort of your home while also collaborating seamlessly with our onsite rehabilitation team. You will maintain regular communication with medical staff, fellow social workers, and support teams to ensure a coordinated approach to patient care. While you can manage your schedule primarily during standard business hours, there may be occasional evening or weekend shifts based on patient needs.
Key Responsibilities- Conduct thorough psychosocial assessments using interviews, direct observations, and extensive review of medical histories to identify emotional, social, and environmental factors affecting patient recovery.
- Provide empathetic counseling and emotional support to patients and their families, addressing adjustment issues stemming from their condition and fostering emotional resilience.
- Collaborate closely with a multidisciplinary rehabilitation team to formulate individualized treatment plans that cater to each patient's unique psychosocial requirements.
- Assist patients in navigating community resources, including support groups, vocational rehabilitation services, and housing assistance to facilitate their transition back to independent living.
- Offer education and guidance to families regarding available post-rehabilitation services and community programs that promote long-term recovery.
- Engage in discharge planning activities, ensuring smooth transitions for patients moving from the detox center to home or alternative care environments.
The patient population at our detox center typically includes individuals experiencing substance use disorders and co-occurring mental health challenges. In this role, you will be pivotal in addressing the emotional and social determinants of recovery, guiding patients through their healing journey while working to re-integrate them into their communities.
Qualifications- A Master’s degree in Social Work from an accredited program is required.
- Active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
- Strong analytical, communication, and interpersonal skills are essential to support diverse patient needs effectively.
- Experience working within a rehabilitation or medical setting is highly preferred.
- A solid understanding of adjustment issues, coping strategies, and grief management is vital for this role.
