Description The Transitional Care Coordinator, coordinates complex discharges and develops a progressive plan to ensure a safe and effective transition to the next care setting. Optimizes the transition from the hospital by actively managing the case and engaging the patient and family to proactively prepare the patient and care team for the complex needs and transition of care. Following the patients throughout the continuum of care into different facilities and home. Focus will be on medication reconciliation between levels of care and reducing avoidable readmissions. The Transitional Care Coordinator will meet with patients at their different levels of care to provide consistent transition overview. Required Qualifications: * Bachelor's Degree in Nursing * Upon hire: Oregon Registered Nurse License * 3 years acute care hospital experience. Preferred Qualifications: * 2 years experience in one or more of the following areas: * Case Management, * Home Health Services, * Community Services, * Discharge Planning. Why Join Providence? The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.
