JOB SUMMARY:
- Provides diagnostic evaluations and psycho-social assessments to patients with defined mental health problems and who are at risk of or are currently abusing alcohol or drugs.
- Assists patients and their families through individual, group and education conferences to understand, accept and follow treatment recommendations.
- Refers patients and their families to specialized treatment programs in accordance with their needs; monitors and evaluates their progress.
- Provides individual psychotherapy to a full caseload of patients for various mental health issues including individuals that require counseling, e.g. HIV/AIDS, Chemical Dependency.
- Collaborates with the other department Directors to integrate care where needed
- Maintains current and appropriate documentation in patient chart.
- Assists the Director of Behavioral Health in conducting in-service training for Cornerstone Family Healthcare medical staff on identifying and dealing with substance abuse, depression screening and other mental health illnesses.
- Acts as a mandated reporter in suspected child abuse cases.
- Provides updates to the Director of Behavioral Health on a monthly basis.
- Rotate on-call schedules with other counselors ***
- Attends monthly Continuous Quality Improvement and Clinical Operations Committee meetings
- Attend and participate in monthly department, All Staff and other required meetings.
- Be familiar with and adhere to all clinical, administrative, and general policies and procedures as outlined in the Cornerstone Family Healthcare Policy and Procedure Manual and Employee Handbook.
- Maintain confidentiality of all aspects of Cornerstone Family Healthcare including, but not limited to, patient confidentiality, financials, and employee relations.
- Perform other duties as assigned.
- Masters Degree in Social Work, Psychology, Counseling or Nursing, certified by New York State where applicable.
- LMSW/LCSW/LMHC is required.
- Minimum 3 years post-graduate experience preferred.
- Experience working with children and adolescents preferred.
LMSW $70,000-75,000
LCSW $80,000-82,000
Salary Description
