Work Type: 0.5 FTE (part-time, 40 hours biweekly)

SUMMARY
Manages caseload responsibilities and provides discharge planning services to patients and their families. Provides counseling and support to patient and families.

QUALIFICATIONS
1. Requires a bachelor's degree in social work, sociology, psychology, or a related field. (Candidates with a master's degree will be hired into a master level position.)
2. Prior social work experience is preferred.
3. Hospital case management experience is preferred.
4. Work requires use of clinical documentation software, so previous experience with electronic medical record systems is preferred.
5. Work requires intermediate computer literacy skills and keyboard proficiency to operate in multiple windows and manipulate files. Work requires intermediate skills in Microsoft Word and email.
6. Pennsylvania state police criminal record check, child abuse history clearance and an FBI criminal history check required.
