Responsibilities:

Position Summary:

Working under the direction of the RN Case Manager, the Discharge Planner completes referrals for post-acute services for patients and assists with tasks that do not require a clinical license or degree.

Required Qualifications:

  • Must possess the skill, knowledge and ability essential to the successful performance of assigned duties
  • Must demonstrate customer service skills appropriate to the job
  • Excellent written and verbal communication skills in English
  • Ability to maintain a work pace appropriate to the workload
  • Ability to establish and maintain effective working relationships across the Health System

Preferred Qualifications:

  • Knowledge of University and medical center organizations, policies, procedures and forms
  • Previous work experience in the healthcare industry
  • Previous work experience with healthcare insurance
