Description
Responsible for providing case coordination and a full range of social work services for non-medical issues, including psychosocial assessment, treatment planning, utilization management, therapeutic interventions, discharge planning, crisis intervention, referral and resource, and acts as a liaison between USFHP Medical Director, Director of Operations, Case Managers, members and Medical providers including Skilled Nursing Attending.

Required qualifications:
- Bachelor's Degree in Social Work, from an accredited school.Training in using efficient, focused treatment approaches to patient problems.
- 5 years of Relevant to job.
- Experience in using efficient, focused treatment approaches to patient problems.in the provision of outpatient community services and discharge planning.
Preferred qualifications:
- Master's Degree in Social Work, from an accredited school.
- Upon hire: Licensed, registered, or certified by the State of Washington as required for that discipline

About the Team
Providence Clinical Network (PCN) is a service line within Providence serving patients across seven states with quality, compassionate, coordinated care. Collectively, our medical groups and affiliate practices are the third largest group in the country with over 11,000 providers, 900 clinics and 30,000 caregivers.
