Description
The County of Yolo is recruiting to fill one ( 1 ) regular, full-time position in the class of Behavioral Health Case Manager I at the Health and Human Services Agency (HHSA) housed in the Child, Youth & Family branch. The Behavioral Health Case Manager is responsible for coordinating and facilitating access to appropriate behavioral health programs and services for youth. This position includes documenting case notes, participating in multidisciplinary team meetings, and collaborating with professional staff to monitor and support youth progress. The coordinator will also assist youth and their families in obtaining necessary supportive services and may visit youth in the home or other locations to provide assistance.
The ideal candidate for this position will have:

Definition
Under supervision, performs paraprofessional tasks related to the treatment of consumers with behavioral illness, emotional disturbance, and/or substance use disorders related to the behavioral health illness or emotional disturbance; provides liaison between professional staff, consumers and community agencies; and provides referral, advocacy, and support services.
Distinguishing Characteristics
The Behavioral Health Case Manager I/II is a single class with two salary levels, and positions in this class are flexibly staffed. Incumbents provide case management services related to the consumer's behavioral health service needs, including but not limited to linkage, referral, advocacy, monitoring signs and symptoms of behavioral health stability as directed by a licensed/waivered clinician, licensed medical staff, clinical manager, etc.
The Behavioral Health Case Manager I is the entry level in this class series. Under close supervision, incumbents are given detailed instructions in the performance of routine tasks that are more structured and repetitive than those assigned at Level II. As requisite skills and knowledge are developed, incumbents are expected to perform increasingly responsible and difficult assignments. Incumbents may advance to the higher level after demonstrating the ability to perform the full journey level duties of the class, which generally requires twelve months of experience at Level I.
The Behavioral Health Case Manager II is the journey level in this class series. This is the targeted competency and performance level of all Behavioral Health Case Managers. Working under general supervision, incumbents are expected to be proficient in performing a variety of responsible and difficult assignments. This class is distinguished from the next higher classification in the job series, Behavioral Health Case Manager III, in that the latter class is the advanced journey and lead level classification requiring the incumbents to possess a Mental Health Rehabilitation Specialist (MHRS) or Certified Alcohol Drug Counselor II (CADC-II) designation.
Essential Functions
Duties may include, but are not limited to, the following:
- Conducts program orientations in group settings or individually with consumers; conducts group meetings, trainings and/or therapy sessions.
- Helps coordinate and facilitate linkage to appropriate substance use and behavioral health programs and services; advises consumers of their rights, responsibilities and eligibility for program participation; explains rules, regulations and policies to consumers and the general public.
- Writes case notes and prepares other appropriate documentation in both written and electronic formats including the AVATAR program; updates consumer treatment plans; prepares, maintains and provides program-related written information, statistics, and tracking spreadsheets and reports.
- Participates in multidisciplinary team meetings; discusses consumer observations and consumer progress with professional staff as input to treatment review.
- Conducts in person interviews with consumers to complete intake packets information which includes consumer's social, medical, psychological, substance use, legal, financial and personal history; Discusses services available to consumer including supportive services.
- Makes referrals to and/or assists consumers in obtaining supportive services including services to meet basic needs such as safety, food, shelter, clothing, medical care, etc.
- Monitors consumer compliance with treatment plan; sets up and maintains controls to ensure timely actions and quality case management.
- Observes attitudes and behaviors to help consumer identify consistencies and confront inconsistencies, identifies and addresses barriers that may prevent consumer from obtaining/maintaining recovery and/or self-sufficiency; counsels consumers to encourage their responsibility in achieving realistic long-term sobriety and/or behavioral health goals.
- Teaches consumers life skills by assisting with banking, grocery shopping, household budgeting, hygiene, etc.; coordinates, facilitates and participates in group outings for the purposes of community integration and education.
- Attends court proceedings with consumers or to testify if subpoenaed.
- Visits consumers in home or other location as necessary; transports consumers when necessary; calls consumers with appointment and/or medication reminders.
- Provides conflict resolution in emergency/stressful situations requiring immediate attention.
- Performs outreach and community engagement functions such as promoting and providing special presentations to outside agencies, schools, and community groups and acting as a liaison with local community and law enforcement agencies.
- Performs related duties as assigned.
Employment Standards
Education: Graduation from high school or possession of a valid G.E.D. certificate; AND
Experience: One (1) year of full-time, paid or volunteer work experience in a behavioral health treatment setting requiring extensive public contact.
Note: Proof of registration of certification as an Alcohol & Drug Counselor, pursuant to 9 CCR § 13000 et al is required by positions within this class performing substance use duties, in addition to the education and experience requirements.
License
Positions in this classification require incumbents to possess and maintain a valid California driver's license, Class C or higher, to carry out job related duties. Individuals who do not meet this requirement due to a physical or mental disability may request a reasonable accommodation.

- Use sound judgment to ensure personal safety and the safety of others
- Effectively manage a diverse caseload, including organizing and maintaining appropriate case records
- Work independently and effectively prioritize work
- Assess a situation and/or problem in order to identify alternative solutions and determine an effective course of action
Incumbents will perform work inside of buildings and outside in the community, and may cross county lines as directed, to provide services; incumbents work alone or closely with others; incumbents must have ability to deescalate violent individuals; must be able to communicate verbally with co-workers and consumers in a variety of situations.
Medical Class
Medical Class I: This class includes administrative or clerical positions requiring light physical effort, which may include frequent lifting of up to ten pounds and occasional lifting of up to twenty-five pounds. Ability to place or retrieve items at above and below waist level will be required. Work is performed while sitting or standing, and considerable moving about will be involved. Incumbents must have corrected hearing and vision to perform the essential functions of the job. Additionally, positions require manual dexterity and eye-hand coordination to use a variety of office equipment such as computers, telephones, copiers, etc.
