As a Home Health Licensed Practical Nurse (LPN), heres a big-picture view of what youll do:
* Provide home-based care to patients with illnesses, injuries and chronic diseasesoften after hospital dischargeunder the supervision of an RN.
* Work with a team of physicians, RNs, CNAs and aides to deliver quality patient care.
* Take patient vitals, do assessments, document progress and report changes to RN supervisor.
* Assist RN in carrying out patients plan of care and teaching patient self-care techniques.
* Educate family on the patients plan of care, medication and home safety.
* Collaborate with interdisciplinary team to coordinate patient care and plan for discharge.
A few must-haves for Home Health Licensed Practical Nurses:
* Graduate of a practical (vocational) nursing program for LPN/LVN
* Current license as an LPN/LVN in the South Carolina.
* At least one-year or 1200 hours of experience as an LPN/LVN.
