This role involves providing high-quality, patient-centered hospice care to terminally ill patients and supporting their families through the end-of-life journey. The ideal candidate will have a strong commitment to palliative care and be skilled in symptom management, patient assessment, and interdisciplinary collaboration. Responsibilities Conduct comprehensive assessments of hospice patients and develop individualized care plans. Manage and monitor symptoms to provide comfort and improve quality of life. Prescribe medications and treatments in accordance with hospice care protocols. Collaborate with interdisciplinary team members including physicians, nurses, social workers, and chaplains. Educate patients and families on illness progression, symptom management, and end-of-life care decisions. Participate in family meetings and provide emotional support to patients and their loved ones. Ensure accurate documentation and compliance with regulatory standards. Provide after-hours support on a rotating basis as needed, consistent with full-time responsibilities. Requirements Current Nurse Practitioner license in the state of Georgia. Certification as a Nurse Practitioner with Hospice and Palliative Care experience preferred. Minimum of 2 years clinical experience in hospice, palliative care, or related field. Strong interpersonal skills and ability to work effectively in a multidisciplinary team. Compassionate attitude with excellent communication skills.
