There’s not a local agency that compares to us. As a new Homecare Physical Therapist or Occupational Therapist, you’ll receive a thorough orientation and mentorship to be successful, as well as strong management support.

Our Therapists are responsible for managing a caseload of patients requiring home-based therapy, primarily post-acute or subacute. Cutting-Edge Technology: Embrace the latest in technology to enhance your impact and efficiency in patient care.
From comprehensive training to providing necessary protective equipment, we're committed to creating a secure environment for you as you deliver exceptional care in our clients' homes.

Promote/provide physical therapy and/or occupational therapy services in the home to assist the patient in achieving his/her optimal level of functioning.
Evaluate and treat individuals with functional deficits secondary to neurologic, orthopedic and medical conditions.
Teach patients/caregivers and help them to become independent with their treatments and self-management of their chronic illness(es)
Coordinate an interdisciplinary plan of care with nursing/physical/occupational/speech therapists, medical social workers, aides and dieticians.
Minimal weekend and holiday rotations

Current NYS PT or OT license and registration
Minimum of one (1) year of experience as a PT or OT, preferably inpatient or homecare, is required
Home care experience preferred, but not required
This position provides mileage reimbursement.

Join us to shadow one of our physical therapists. Physical Therapist Pay Range: $37.60 - $48.10
Occupational Therapist Pay Range: $35.85 - $45.96
Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
