Position Overview
Aveanna Healthcare has experienced significant growth over the past year through strategic acquisitions, leading to an increased demand for skilled professionals in our dynamic home health care team.
We are seeking a qualified Physical Therapist (PT) to join our team and play a vital role in enabling patients to recover from illness or injury in the comfort of their own homes.
Key Responsibilities:
Perform thorough assessments to evaluate patient mobility, coordination, strength, and current level of function.
Evaluate and interpret test results to determine functional goals and outcomes.
Develop written plans of care outlining physical therapy needs, goals, progress, and expected outcomes.
Deliver high-quality physical therapy services in accordance with prescribed physician orders.
Maintain accurate records of client performance, modify treatment plans as needed, and determine discharge care arrangements.
Collaborate with healthcare teams, physicians, nurses, and family members to ensure seamless care coordination.
Document patient information according to company policies and procedures.
Serve as a resource consultant for clinical staff and maintain effective communication between team members and families.
Requirements:
Degree in Physical Therapy from an accredited university/program.
Current, unrestricted state license as a Physical Therapist in the state of practice.
Valid CPR certification.
