Job Summary and Qualifications TheCNC/CNL uses the nursing process and evidence-based practice to work collaboratively with an assigned group of patients andcareteam in developing the patient-driven holistic care plan for life. TheCNC/CNL, as a member ofa larger team, promotes patient and family health education with a focus on self-management, prevention, and wellness, based on the patients goals. TheCNC/CNLis responsible forthe lateral integration of patient care, i.e., assessing patient needs and accessingexpertiseacross healthcare disciplines. Fundamental aspects of theCNC/CNLroleincludeleadership in the care of patients; standardization of care; identification, delegation and oversight of care delivery and outcomes. TheCNC/CNL refines systems toidentifytrends and opportunities for improvement, incorporating efficiency and cost-effective care with well-defined patientoutcomes. Assists with admission and discharge processes to ensure efficient throughput and high quality, patient-centered care. Participates in the ongoing assessment of the quality of patient care services provided in the unit, in collaboration with other members of the management team. Supports the efforts of the facility to improve engagement by operationalizing current nursing strategies, including employee rounding, hourly rounds, and other initiatives. Provides recommendations related to interviewing, selecting, and training new staff. Recommends and implements courses of action, including training and development, conflict resolution, personnel policy compliance, completion of performance evaluations, and/or disciplinary actions to ensure a competitively better organization. Manages all practical aspects of staff labor in accordance with patient care needs and established productivity guidelines.
