MEDICAL NURSE PRACTITIONER

Nurse Practitioner (Home Visits) - $140k-$150k annual salary An innovative tech-driven healthcare company located in Monmouth County, NJ is currently seeking a Nurse Practitioner to provide Home Visits to patients. In this role, you will provide personalized, high-quality care to patients in their homes while enjoying a manageable patient load and the support of advanced technology . Prior experience in primary care or home care required . This is a full-time direct hire opportunity. Paid medical insurance and PTO offered. Key Responsibilities:
Deliver comprehensive, patient-centered healthcare services in the comfort of patients homes.
Perform assessments, diagnose conditions, develop treatment plans, and provide follow-up care.
Leverage advanced technology to streamline workflows, enhance patient care, and document visits efficiently.
Collaborate with interdisciplinary teams to ensure continuity of care.
Educate patients and families on managing chronic conditions, medications, and lifestyle adjustments.
Provide compassionate and respectful care, fostering trust and strong relationships with patients.
Requirements:
Active Nurse Practitioner (NP) license in NJ.
Minimum of 1 experience in primary care, home care, or a similar setting.
Exceptional interpersonal and communication skills.
Tech-savvy with the ability to utilize digital tools for patient care and documentation.
Passion for providing high-quality, personalized care.
Please note- The annual base salary range is $140k to $150k. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
