Job Description· Support our patients, families, and loved ones, as they go through their end of life journey· Perform a comprehensive assessment to establish goals for symptom management and end of life care with effective case management strategies· Be a member of an active interdisciplinary team that addresses patients' needs holistically· Develop and maintain a therapeutic relationship by utilizing effective communication with patients and their families.· Utilize state-of-the-art technology to document skilled services provided.
