Primary Care Nurse Practitioner (Bridging Care Provider) Deerfield Beach, FL COMPENSATION: Salary $150k + $10k Bonus We are a unique physician-led culture that provides coordinated, world-class primary care to the most vulnerable population, moderate to low-income seniors with complex chronic diseases. Our approach enables us to drive key quality and cost outcomes that create value for patients, physicians, and the overall health system. We are able to practice medicine the way it should be practiced. We build relationships with our patients the old fashion way, we have a reduced patient panel that allows us to see them with a higher frequency. Simply put, we give our patients the time and respect they deserve and require. Role: Compensation: $150k Bonus: Up to 10k year 1 and goes up as a partner. 18-20 patients a day Hours: Monday Friday work schedule; Clinic Hours 7:30a 5:00p (last patient is scheduled at 4:15) Work/Life Balance Working Multiple locations covering for clinicians on PTO and overflow of patients Administrative time included in schedule No nights or weekends 100% outpatient Dedicated Care and support team (have your own MA/Scheduler/ Patient Population: Seniors with 3-5 chronic diseases Value Based Care Outpatient Benefits: Paid Time Off Comprehensive Benefit Package Attractive Health, Dental, Vision and supplemental benefits plans for the provider in their family 401K 5.5% match Clinic offers: Inhouse Consulting Specialist: Cardiology, Podiatry and more Holistic Health Services: Acupuncture, Tai Chi, Yoga and Nutrition Door to Door Transportation Requirements: Board Certified Family Medicine or Geriatric Medicine Bilingual: Spanish Speaking (Preferred)2+ years NP Primary Care outpatient experience Deerfield Beach combines a steady primary care market with beautiful beaches, year-round sunshine, affordable coastal living compared to nearby Boca and Miami, and easy access to major South Florida amenities.
