Job Overview RN Case Manager The Case Manager (RN) is responsible for coordinating care for health plan members by using a collaborative process of assessment, planning, facilitation, and advocacy. The goal is to help members access the right services at the right time while ensuring care is clinically appropriate, cost-effective, and aligned with their benefit plan. This role requires an active, unrestricted Registered Nurse (RN) license in the state of Michigan (MI). The Case Manager uses clinical tools, data, and established guidelines to complete assessments, develop care plans, coordinate services, and monitor outcomes, all while working within regulatory, accreditation, and organizational policy standards. Key Job Duties / Responsibilities Conduct comprehensive clinical assessments of referred members using clinical tools, data review, and member/family/provider input. Determine member eligibility, needs, and appropriate approach to case resolution based on the members benefit plan and available internal and external resources. Apply and interpret case management criteria, guidelines, standardized case management plans, policies, and procedures to ensure appropriate administration of benefits. Develop, implement, and monitor individualized care plans to support member health goals and promote optimal, cost-effective outcomes. Coordinate services across providers, facilities, and community resources to ensure timely and appropriate care delivery. Utilize case management and quality management processes in compliance with regulatory, accreditation, and organizational requirements. Document all assessments, care plans, interventions, and member interactions accurately and timely in the appropriate systems. Communicate effectively (verbal and written) with members, families, providers, and internal stakeholders to support care coordination and issue resolution.
