Job DetailsNurse Practitioner (Family and/or Adult APRN Certification), Physician Assistant- Endocrinology IThe Cleveland Clinic Weston Endocrinology Department is seeking a team oriented and engaged Advanced Practice Provider to join our team of extremely supportive caregivers, serving the adult Endocrinology population in an outpatient setting. Duties/procedures involved include
•Conducts thorough medical histories, performs complete physical examinations (where indicated), initiates appropriate lab, radiology tests or other special tests required for evaluation of illness, and scrutinizes lab data on new and former clinic patients and hospital admissions to establish a record of the patient's current health status and to develop a working diagnosis and treatment plan.
•Performs and interprets, at least to the point of recognizing deviations from normal, common laboratory, radiologic, and other routine diagnostic procedures used to identify pathophysiologic processes as privileged by CCF Credentialing Board.
•Performs routine procedures managing simple conditions assisting in the management of more complex illness and taking initiative in performing evaluation and therapeutic procedures in response to life- threatening situations, as privileged by CCF Credentialing Board.
•May prescribe and monitor medications as allowed by prescriptive authority through applicable state Board of Nursing/Medicine and CCF policy.
•Performs specialized management of patients using basal/bolus insulin shots, insulin pumps, and continuous glucose monitors (CGM) and other diabetes related therapies.Schedule: Monday - Friday. Combination of in-office and virtual duties. Occasional diabetes-related inpatient duties with protected time.Endocrinology Experience preferred, although newly licensed considered, especially with endocrinology experience or clinical rotations.Responsibilities:Conducts thorough medical histories, performs complete physical examinations (where indicated), initiates appropriate lab, radiology tests or other special tests required for evaluation of illness, and scrutinizes lab data to establish a record of the patient's current health status and to develop a working diagnosis and treatment plan. Works in collaboration with the health care team.Performs and interprets common laboratory, radiologic, cardiographic and other routine diagnostic procedures used to identify pathophysiologic processes as credentialed and privileged by the Medical Executive Committee and the Governing Body of the applicable hospitals.Performs routine and specialized procedures as credentialed and privileged by the Medical Executive Committee and the Governing Body of the applicable hospitals.May prescribe and monitor medications as allowed by license through applicable state board and Cleveland Clinic policies.Participates in medical team rounds; collaborates with nursing, medical and other healthcare team members regularly to ensure quality patient care.Refers patients to specialists as appropriate for consultation or for specialized health resources and treatment.Ensures continuity of care by serving as a liaison between patient and other members of the multi-disciplinary care team or with other specialty areas as necessary.Participates in quality monitoring thru the review of records and treatment plans for patient outcomes on a periodic basis to assure quality care.Facilitates appropriate length of stay, discharge planning and compliance with regulatory standards for inpatient management.Supports professional development of the health care team.May assists in data collection and analysis.May serve as clinical preceptor for students.
