Primary Care Nurse Practitioner

Nurse Practitioner (NP) – Long-Term Care Location: Schedule: Monday–Friday | 8:00 AM – 5:00 PM EST
Work Schedule: 4 days per week (flexible; We are seeking a compassionate and experienced Nurse Practitioner (NP) to provide high-quality primary and preventative care to residents in skilled nursing facilities in the Defiance, Ohio area. This role focuses on long-term care, chronic disease management, and collaboration with interdisciplinary teams to ensure comprehensive, patient-centered care.
Provide primary and preventative medical care to residents in skilled nursing facilities
Assess and manage residents experiencing acute changes in condition
Perform advanced care planning and chronic care management
Communicate regularly with nursing home staff regarding resident status and care plans
Collaborate with primary care physicians and interdisciplinary care teams
Maintain accurate and timely documentation within the electronic medical record (EMR)
Current, unrestricted Nurse Practitioner license in the state of Ohio
Board Certification as a Nurse Practitioner
Clinical background in Adult, Family, or Geriatric medicine
Prior experience working in a long-term care or skilled nursing setting
Proficiency with electronic medical records (EMR systems)
Strong collaboration skills within an interdisciplinary care team
Psychiatric experience is a plus
Commitment to patient-centered, compassionate care
