Job Summary:
Care Advantage, Inc. is seeking multiple Licensed Practical Nurses (LPN) to support our growing High Acuity Division in Virginia.
We provide one-on-one care in clients' homes, aiming to deliver the best possible care to our patients.
Our goal is to reduce depression and mitigate cognitive impairment through specialized care plans developed by Registered Nurses, ensuring our patients' safety and comfort at home.

Responsibilities:
Provide direct patient care and implement personalized care plans
Maintain accurate and timely documentation consistent with the plan of care
Assess and educate family members or caregivers on patient care
Support RNs and physicians in performing specialized procedures
Coordinate home health services and report identified needs to the registered nurse and/or Clinical Supervisor
Requirements:
Eligible to work in the United States
CPR Certification and PPD Certification
At least 1 year of experience as an LPN
Valid LPN License with at least 1 year of work experience
Valid Driver's license and reliable transportation
