Gateway Home Health Corp. is a beacon of compassionate and innovative home health care. Founded in 2016, we are an independently owned and operated agency, born from the combined vision of a dedicated Home Health physical therapist and her husband, a retired Air Force captain. Our journey began with a commitment to providing empathetic and professional care to those who need it most, right in the comfort of their homes. In 2017, Gateway Home Health achieved a significant milestone by becoming Medicare-certified and Joint Commission-accredited, affirming our dedication to the highest standards of care. The following year marked another leap forward as we welcomed a new partner with extensive expertise in Home Health, Hospice, and information technology. This strategic expansion was not just about growth; it was about enriching our services with cutting-edge technology and adapting to the ever-evolving standards of the health industry.

Our people are our greatest asset so we aim to pay above market rate for all positions, and we pay clinical staff for all drive-time between patients. We also provide medical, dental, paid time off, and a 401(k) with full 1-1 match for full time employees.

Responsibilities:

-Provides physical therapy services to patients according to a written physician's plan of care. This may include, but will not be limited to:

A. Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. Evaluates home environment.

B. Assesses for muscle strength, mobility, gait, ROM-potential for rehab.

C. Directing physical therapy treatment.

D. Instructing patients and families/caregivers in the use and care of therapeutic appliances.

E. Determining priority needs for physical therapy

- Evaluate and assess patients' physical conditions and develop appropriate treatment plans

- Implement therapeutic exercises and techniques to improve patients' mobility, strength, and range of motion

- Provide manual therapy techniques such as trigger point therapy to alleviate pain and promote healing

- Provides direct physical therapy according to directions of physical therapist and in accordance with the physician's plan of care.

- Treats patient to relieve pain, develop or restore function, and maintain maximum performance.

- Directs and aids patients in active and passive therapeutic exercises, muscle reeducation, gait and functional training, transfer activities and prosthetic training.

- Educate patients and their families on proper techniques for managing their conditions and preventing further injury

- Collaborate with other healthcare professionals to coordinate patient care and ensure optimal outcomes

- Observes and reports to the physical therapist and the Organization personnel the patient's reaction to treatment and any changes in patient's condition.

- Instructs patients in care and use of wheelchairs, braces, crutches, canes, and prosthetic or orthotic devices.

- Maintains necessary records; Prepares and submits a clinical progress summary based on the attainment of goals as directed by Organization policy (within 48-hours)

- Participates in Organization Home Care Team Members meetings and Organization Inservice Education Program.

- Maintain accurate and up-to-date patient records

-When therapy is the only skilled service, instructs, supervises and evaluates home health aide care every two (2) weeks.

- Participates in peer consultation process.

- Supervises Physical Therapy Assistants according to organization policy and state regulations.

Experience and Qualifications:

- Graduate of an accredited physical therapy assistant program with either an Associate's degree in science or applied science that is approved by American Physical Therapy Association, Inc.

-Licensed to practice as physical therapist within the State, if applicable.

- Possesses and maintains current CPR certification.

- Current certification or license as required by state law.

- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

- Minimum of 2 years of experience as a Physical Therapist

- Strong knowledge of anatomy, physiology, and medical terminology

- Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare professionals

- Ability to work independently and make sound clinical judgments

- Proficient computer skills for documentation and data management

- Experience with Synergy EMR is preferred but not required

- Experience documenting accurate OASIS data is preferred but not required

- Understanding of the PDGM Medicare model

Skills:

- Excellent patient care skills, including empathy, active listening, and effective communication

- Proficiency in conducting thorough evaluations and developing individualized treatment plans

- Ability to adapt treatment techniques based on patients' needs and progress

- Strong manual therapy skills, including trigger point therapy

- Knowledge of evidence-based practice guidelines and ability to integrate them into treatment plans

- Experience working with geriatric patients is a plus.
