Job Description:

Responsibilities:

Assess, diagnosis, and provide treatment for patients using the DSM-5, for both

substance abuse and mental health needs.

• Collaborate with an interdisciplinary team to develop appropriate supports and

interventions, discharge planning, service coordination.

• Demonstrate proficient skill in evidence-based practices and theories to work with

adults, children, and their families.

• Screens admissions for high risk patients who need social service intervention.

• Assesses patients and families' abilities and/or needs they may interfere with

rehabilitation and medical progress.

• Assists patients and families to fully utilize community resources.

• Documents social work intervention in medical records within timely manner.

• Coordinates discharge planning and facilities discharge planning rounds.

• Coordinates organ/tissue donation progress.

• Oversees advanced directives procedures.

• Counsels patient/family to achieve optimal level of psychosocial functioning

Required:
Day 5x8-Hour (07:00 - 15:00)

• Special requirements: Position requires Oregon license in hand. Would entertain a SW or RN Care Manager. 4 days/week will be at PNMC, 1 day/week they will float to another ministry.
