Pacific Crest Hospice of Ventura provides professional medical care and volunteer support to people with life limiting illness. Our goal is to provide excellent care so that our patients remain comfortable in their own homes and stay as independent as possible.

Pacific Crest Hospice of Ventura is recruiting experienced and compassionate hospice professionals and is currently accepting applications for a per diem RN with option for growth to part-time or full-time for the right candidate for field visits (including on-call shifts)

*General Responsibilities:*

* Assessment, creation and management of patient plan of care
* Ongoing reassessment of all patient needs (to include psychosocial & spiritual if necessary) and updates to plan of care as relevant
* Participates in QAPI activities
* Takes on-call duty, nights, weekends and holidays as assigned
* Follows federal, state, local and accreditation standards for patient care
* Care provided in patient homes, RCFEs (assisted livings and board and cares), nursing homes
* Other duties as assigned

*Ideal Candidate Skills/Knowledge:*

· Graduate for accredited school of nursing and current California RN license (required)

· Minimum 1 year of experience as a professional nurse in past 3 years (required)

· Hospice or Home Health experience (preferred)

· Valid California Driver's License and reliable transportation

Proof of vaccination or request for exemption must be submitted at the pre-employment physical, if applicable, but not later than the day prior to the anticipated start date.

Pay: $45.00 - $50.00 per hour

Education:
* Associate (Required)

Experience:
* Hospice & palliative medicine: 1 year (Preferred)
* nursing experience as a professional nurse: 1 year (Required)

License/Certification:
* California RN license (Required)

Shift availability:
* Day Shift (Preferred)
* Night Shift (Preferred)
