REPORTS TO: Chief Medical Officer and Supervising Physician
EDUCATION & EXPERIENCE: BA/BS, graduate of AMA accredited Physician Assistant program.
Current or pending PA license, current BLS certification.
SKILLS: Provide professional health care services for clients in an ambulatory clinic setting.
PRIMARY DUTIES:
conduct Annual Wellness Visit (AWV) visits for Medicare patients following established guidelines.
develop and implement personalized care plans based on AWV findings.
collaborate with the healthcare team to integrate AWVs into the overall patient care strategy.
provide general physical, medical evaluations and screening to clinic patients.
share responsibility in joint practice collaboration with the Medical Director and other Center health caregivers in providing on-going supervision of patient treatment modalities, progress and compliance.
provide diagnostic, treatment, and preventive medicine as indicated by protocol and/or Medical Director's consultation, in accordance with applicable laws and regulations.
use appropriate clinic judgment in treatment of patients in emergency situations when the physician is not immediately available. Such treatment is followed by physician consultation and/or referral as soon as possible.
make appropriate specialty referrals when necessary, seeking prior consultation with on-site physician if needed.
abide by policies and procedures as dictated in the Center's various manuals, including but not limited to its Personnel, Safety and Infection Control manuals.
participate in the agency's Compliance and Performance Improvement Program.
OTHER DUTIES:
assist in the Center's provision of health education and health promotion programs including but not limited to such groups as the Center's patients, physicians, staff, and/or community residents.
other duties as assigned by the Medical Director and/or Center management.
