Overview:

LPN - Split Shifts with Every Other Weekend - 33/44 Hours/Week

Job Description:
We are seeking a skilled LPN to provide specialized in-home care for a patient in the Newbern area. This role involves caring for a patient with both a trach and G-tube, requiring your expertise and attention to detail.

Schedule:

Weekdays: 7:00 AM to 6:00 PM
Every Other Weekend: Split shifts, same hours

You’ll be working an average of 33 to 44 hours per week, depending on the rotation.

Key Responsibilities:

Provide comprehensive care, including trach and G-tube management, along with medication administration and health monitoring.
Ensure accurate and timely documentation of patient care.
Deliver compassionate, patient-centered care in a home setting, maintaining the highest standards of safety and quality.

Qualifications:

Current LPN license in Tennessee.
Experience with trach and G-tube care is required.
Strong attention to detail and the ability to work independently.

Responsibilities:
Perform selected nursing procedures within the scope of LPN practice
Provide patient and family teaching according to plan outlined by the RN
Monitor patient's vital signs and make specific measurements and observations
Monitor and record intake and output. Report significant changes immediately
Observe, assess and record patient's physical and mental condition and reports any changes
Administer medications as instructed and as ordered by the physician and instructed by the RN and document the patient's file accordingly
Attend and participate in multi-disciplinary patient case conferences, staff meetings and in-service education programs as assigned
Qualifications:
Licensed Practical Nurse / LPN license in good standing
Previous private duty, home health, hospice, hospital or skilled nursing experience preferred
Active CPR certification
Familiarity with electronic health records
Strong problem solving and critical thinking skills
Reliable transportation, valid driver's license and current automobile insurance
