Responsibilities of the Medicare Nurse involve :

  • Provides Medicare visits for HHC patients
  • Is familiar with Medicare requirements and is able to perform within a given
    scope of practice
  • Provides ongoing support and evaluation of progress toward stated health goals
    Revises the healthcare plan as needed in collaboration with the patient and
    healthcare team.
  • Possesses excellent communication and collaboration skills.
  • Determines the need for and completes appropriate referral processes
  • Maintains EHR documentation and data. Completes all documentation in a timely manner
  • Meets with supervisor regularly to provide patient updates


Qualifications:
Graduate of an accredited School of Nursing (BSN or equivalent Preferred).
Active Registered Nursing license in Maryland / Virginia
Current CPR certification
Clinical experience: 1 year (minimum) direct patient care within the last 3 years preferred
Candidate possessing strong clinical skills to manage the care of technologically dependent patients with trachs / feeding tubes / ventilators is preferred
