Description

This position is a licensed RN, PT, OT or SLP with the skills and abilities to perform home health staff RN or Rehab duties. The Home Health Clinical Manager serves as a communication hub responsible for coordinating the multidisciplinary home health services for the clinical team. This position acts as a liaison between Access Data Entry, Scheduling, Quality and clinical Staff. This position provides communication, information and the tools necessary to maintain best practices in all team roles. The Home Health Clinical Manager will have access to confidential organizational financial data pertinent to OASIS data integrity to compile data and/or reports utilizing both workflow and financial planning information.

Required Qualifications:

  • Bachelor’s Degree in Health care related field.
  • Washington Registered Nurse License upon hire.
  • National Provider BLS - American Heart Association within 30 days of hire.

  • 3 years In the nursing or rehab field.

Preferred Qualifications:

  • Case Management.
  • Supervisory Experience in Home Health.

About the Team

Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.

HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses.
