As aLicensed Practical Nurse (LPN) Visit Nurseyou will:

  • Providein-homedirect patient care according to theprovidersprescribed plan of care (and nursingscope of practice) while closelymonitoringprogress/recovery
  • Complete skilled assessments and provideexceptional patient-centered care in the home
  • Ensure an accurateevaluation and implement a comprehensive care plan that addresses patient needs
  • Communicate patient conditions and collaborate with appropriate providersto deliver care when patient needs evolve
  • Complete all clinical documentation in accordance withagency protocol and Medicare/Federal guidelines
  • Provideeducation to patients and family members involved inpatientscare.
  • Work to decrease readmissions by promoting preventative care and ensuring continuity of care

To qualifyfor aLicensed Practical Nurse (LPN)positionwith us, you will need:

  • Graduate of a practical (vocational) nursing program for LPN/LVN and has a current license as anLPN/LVN in the state(s) in which he/she practicesand have atleast1200 hoursof experience as an LPN/LVN or completion of the Office's preceptor program
  • Licensure: Current unrestricted license to practice as Licensed Practical Nurse (LPN)in the state associated with this position
  • Current CPR/AED/BLS/First Aid certification
  • Reliable transportation to/from care sites and/or work locations
  • Two (2) years of experience practicing as a Licensed Practical Nurse (LPN in a home health or similar setting
